Over the summer, Kanye West's Yeezy design studio in Calabasas, California was the site of a workplace accident when an employee was said to have dropped a 3D printer on his foot. Now, OSHA's findings are shedding more light on the incident.

A new report from the California Division of Occupational Safety and Health details the injury, in which a worker's left hand and foot "sustained crush injuries."

According to its filing, the accident occurred when two employees were using a pallet truck to relocate a sole press machine, which is presumably the 3D printer named in prior reports. While the two workers were operating the machinery, the truck and sole press tipped over, falling on one of the men. 

OSHA has ruled that the truck was being used on an inclined surface which went against manufacturer precautions. Furthermore, it's also stating that the employer failed to provide proper information and training for the safe use of the equipment. 

As a result, OSHA has proposed two fines against Adidas America totaling $45,000.

A request to Adidas for comment has not yet been returned.
