Authorities in Utah say a 3-year-old girl fatally shot her her 2-year-old brother with a rifle at their home.

According to the New York Daily News, their mother called 911 after hearing the weapon discharge inside of the family's home in  College Ward, Utah. Minutes later, emergency responders were on the scene to hurry the boy to a local hospital, where he was then transported to Primary Children’s Hospital. Sadly, doctors were unable to save him.

Local police believe the children got a hold of the .22-caliber rifle after their father left it unattended. "The gun was in an unloaded state but did not have live rounds in the magazine," Cache County Sheriff Lt. Mike Peterson explained. "We believe the 3-year-old had to manipulate the action enough to chamber a live round prior to the incident occurring."

Though authorities are allowing the family time to cope with the tragedy, Peterson said an investigation will eventually be conducted.
